This is to inform the general public that applications are invited from suitably qualified candidates into 3 years Basic Midwifery Programme in the School of Midwifery Mater Misericordiae Hospital, Afikpo.

ENTRY REQUIREMENTS
1. Candidates must be at least 16 years of age.

2 Candidates must possess the senior secondary school certificates or its equivalents (SSCE, NECO, NABTEB,GCE) with a minimum of five (5) credits in English language, Mathematics, Physics, Chemistry and Biology in not more than two sittings.

3. Birth certificates/declaration of age

4. Testimonial from the last school attended

5. A letter from your parish priest or pastor

6. Marriage certificate (if married)

7. Original/photocopy of relevant documents will be sighted during interview

8. Three (3) recent passport photographs

9 Baptismal card (very important)

10. Bring any of PVC, NIN, International Passport, Drivers licence

METHOD OF APPLICATION

1. Sale of form continues till 20th March 2024 at the school.

2. Form is obtained on payment of Non-refundable fee of (N12, 550) into the school Account with the ACCOUNT NUMBER: 3031304862, Name of the account: School of Midwifery Mater Hospital, Afikpo, First Bank Of Nigeria PLC.

3. Visit the school with the evidence of payment, change the bank teller to receipt and collect the entrance form.

4. Completed forms should be returned to school on or before 21st March, 2024

5. The courses for the entrance examination include: Mathematics, English Language, Physics, Chemistry And Biology.
